Practitioners applying for inclusion in the Headache Doctor Registry must hold an unrestricted MD or DO license in their practicing jurisdiction and possess recognized fellowship training in headache medicine or board certification in Neurology, Physical Medicine & Rehabilitation, or Anesthesiology Pain Medicine. Submissions must include active NPI numbers, state license details, hospital affiliation credentials, and clinic contact protocols.
